Setting up a care plan begins with filling out a referral form. A representative from Golden Valley Home Care will guide you through a step-by-step process:

What Does Senior Care Coordination Cost?

At Golden Valley Home Care, our senior care coordination services are fully covered by Medicaid. This includes coordination of home care, social support, and non-medical services. However, certain skilled medical services or services from partner agencies may not be fully covered by your state’s Medicaid program or may incur additional costs.
